Action Needed: Annual Health & Medical Record Form (Due September 14th)
It's that time of year again — please help us stay prepared by completing the Annual Health & Medical Record (Parts A & B) for the upcoming scouting year.
Who needs to fill one out:
Every Scout (as well as any parent, guardian, or sibling who will be participating in Pack activities) needs newly completed form on file for the 2026-2027 scout year. We recommend all scouts have at least one parent/guardian form on file as most activities require adult participation. (Adult participation is always required for Lion and Tiger scouts)
Due date: September 14th
How to submit:
- Fill out Parts A & B of the Annual Health & Medical Record
- Scan the completed forms
- Email them to pack972portland@gmail.com
- CC your den leader on the email
